The Position

Under general supervision, the Medical Case Management Nurse performs a variety of responsible medical case management duties; ensures completion of tasks in accordance with established policies and procedures; communicates policies, procedures and job expectations; and provides training to staff.

Knowledge of

  • Principles and practices of leadership, mentoring, and training
  • Applicable federal, state, and local laws, codes, and regulations on nursing practices 
  • Operational characteristics, services, and activities of assigned programs and functions
  • Office procedures, methods, and equipment including computers and applicable software applications such as word processing, spreadsheets, databases, and other specialized applications related to area of assignment
  • English usage, spelling, grammar, and punctuation
  • Professional nursing principles, procedures, and techniques used in the care and treatment of patients with a wide variety of conditions, normal course of illnesses and injuries, and their anticipated complications and therapeutic interventions
  • Medical reference manuals such as the California Relative Value Studies, Physician’s Current Procedural Terminology Handbook, Western Region Length of Stay by Diagnosis and Operation, and Intensification Criteria for Concurrent Utilization Review
  • The Centers for Disease Control Prevention (CDC) – The Epidemiology and Prevention of Vaccine Preventable Diseases
  • General procedures and organization of hospitals and out-patient clinics

  • Learn, use and appropriately apply medical reference manuals, insurance policies and regulations, and various governmental regulations pertaining to medical care and payment related to the program assigned
  • Communicate effectively, orally and in writing, with patients, their families, hospital and clinic personnel and insurance representatives

Employment Qualifications

Minimum Qualifications

Possession of a current valid Registered Nurse license issued by the State of California;

AND

Either: One (1) year of full-time paid experience as a Registered Nurse performing case management activities such as coordinating out-patient or in-patient nursing care, or medical review in a hospital or clinical setting.

Or: Two (2) years of full-time paid experience as a Registered Nurse performing nursing duties. 

Note: If the word “experience” is referenced in the minimum qualifications, it means full-time paid experience unless the minimum qualification states that volunteer experience is acceptable. Part-time paid experience may be accumulated and pro-rated to meet the total experience requirements.

General Qualifications

Criminal History and Background Checks: The County may access criminal history information on candidates who have accepted a conditional offer of appointment for this class consistent with the provisions of Board of Supervisors Resolution No. 82-602, Personnel Policies, and applicable federal and state laws. The County shall not consider for employment any candidate who has been convicted of a felony or misdemeanor that relates to or impacts the candidate’s ability to perform the job duties of this class unless it is determined that mitigating circumstances exist. For purposes of accessing criminal history information, the candidate will be fingerprinted. A subsequent arrest notification may be obtained.

The County may also conduct a background check on the candidate prior to appointment to a position within this class. The background check may include personal and professional reference checks, credit history checks where authorized by Labor Code section 1024.5, Social Security Number verification, professional license/registration verification, military service information and driving history. Information obtained in the course of this background check will be considered by the appointing authority in the selection process. In obtaining such information, the County will comply with applicable consent and disclosure practices in the Fair Credit Reporting Act and the California Investigative Consumer Reporting Agencies Act. 

Nurse License Requirement: In accordance with applicable federal and/or state regulations, incumbents in this class are required to possess and maintain a current valid license issued by the State of California as a Registered Nurse. Failure to maintain the required license may constitute cause for personnel action in accordance with Civil Service rules.

Certification Requirement: A valid Basic Life Support (BLS) issued by the American Heart Association or Cardio-Pulmonary Resuscitation (CPR) issued by the American Red Cross is required at the time of appointment and must be renewed every two years.

Special Requirements: Some assignments in this class require incumbents to possess and maintain a current valid certification as a Public Health Nurse issued by the State of California. Failure to maintain the required license may constitute cause for personnel action in accordance with Civil Service rules. 

Physical Requirements: Some positions in this class require the incumbents to be able to:

  • Sit for extended periods; frequently stand and walk. 
  • Normal manual dexterity and eye hand coordination; corrected hearing and vision to normal range; verbal communication. 
  • Use of audio-visual equipment; use of office equipment including computers, telephones, calculators, copiers, and FAX. 


Working Conditions: Some positions in this class require the incumbents to:

  • Work weekends, holidays, and non-routine hours. Work is typically performed in an indoor office environment and controlled temperature conditions. Tasks may involve extended periods of time at a keyboard or workstation. 
  • Travel to and from other locations in a variety of outdoor weather conditions and transport and lift materials, equipment, and/or supplies weighing 10-25 pounds. 

Probationary Period

The probationary period for this classification is six (6) months.

FORMULA RATE EXAMINATION (Weighted 100%)


All candidates meeting the minimum qualifications by the cut-off date will have their Supplemental Questionnaire scored in the Formula Rate Examination.


This examination will evaluate the relevance, level, recency, progression and quality of candidate's education, training and experience. The candidate's application or other materials will not be included in this examination. Therefore, the candidate's responses to the supplemental questionnaire should be thorough, detailed and complete. The score from the Formula Rate Examination will determine the ranking on the eligible/employment list for this job.


All candidates competing in the testing process will receive written notice of their examination results by email. Notices can also be accessed in their governmentjobs.com inbox. Applicants achieving a passing score will be placed on the eligible list in rank order. The rank is determined by the test score attained from the examination.
