Job Description:
Within Hill Physicians’ care coordination framework, provide telephonic psychosocial assessments and interventions to assist patients and caregivers in making care transitions and coping with their illness, diagnosis, treatment, and/or life situation, including emotional, mental, and substance abuse disorders.
Job Responsibilities
Education of members, family, and care givers around issues related to adaptation to the member's diagnosis, illness, treatment and or life situation. Address biopsychosocial stressors relating to disease management to improve members overall well-being, and in the process, reduce ED utilization and readmissions.
Address barriers to social determinants of health (SDOH) and connect members to community programs to address these barriers.
Coordinate and collaborate with hospitals, skilled nursing facilities, home health, hospice, and other community agencies to advocate for members ongoing care needs.
Participation in inter-disciplinary team meetings to discuss member's needs and address care gaps.
Assist in advance care planning and completion of Advance Directives and POLST. Coordinate palliative care services and/or end-of-life services where appropriate.
Required Experience
LCSW
2-4 years social work experience within the last 5 years (MSW fieldwork not included), preferably in a managed care environment. Hospital experience preferred.

Salary Range
$95,000 - $120,000 Annual
